What are HMO, PPO & POS plans?
Health Maintenance Organization (HMO), Preferred Provider Organization (PPO) and Point-of-Service (POS) plans are types of network-based plans. Here's how they differ:
- HMO plans cover the care plan members receive through a network of local contracted doctors and hospitals. These plans generally do not provide coverage outside the network of contracted providers, except in emergencies.
- PPO & POS plans let plan members choose between in-network and out-of-network providers. These plans cover all medically-necessary covered benefits, whether they're received in network or not. However, services outside the network will generally cost more.

Together, UnitedHealthcare’s Medicare Advantage HMO, PPO & POS plans are available in 70 markets across 40 states, plus Washington, D.C. Our Medicare Advantage Private Fee-for-Service (PFFS) plans are offered nationally.

You can choose from nearly 80 standard combined medical and prescription drug plan designs.
