UnitedHealthcare's Optimum Choice, Inc. HMO plans require members to select a primary care physician who is responsible for providing or coordinating medical care, including in-network referrals for specialty care and hospital care, when medically necessary. UnitedHealthcare's Optimum Choice, Inc. Preferred plans also require a primary care physician and their coordination of care, but provide in-network and out-of-network coverage.

Optimum Choice, Inc. is also available in Health Savings Account (HSA) eligible plan designs. With these plan designs, members can open and deposit money, before taxes, into an HSA, which is a personal savings account. Members can use the money to pay for eligible medical and pharmacy expenses, including their deductible, or save it for future expenses.
