These plans provide access to a national network of over 626,000 physicians, 5,035 hospitals and 60,000 pharmacies. They also offer members access to value-added programs designed to address their overall health and well-being. These new plans can be paired with our other plans expanded dual-choice flexibility.
UnitedHealthcare ValueMax plans control costs by:
Excluding inpatient and outpatient mental health/substance use disorder coverage
Limiting in-network/out-of-network sickness and injury primary care physician/specialist visits to three per year with no coverage after visits are exhausted
An annual policy maximum in lieu of a lifetime maximum
Per member deductible and out-of-pocket maximum benefits (does not include family levels)
Limiting outpatient rehab services to combined in-network/out-of-network and any combination of type to an annual 10-visit maximum.
In-network outpatient surgery will be covered at a coinsurance level 10 percent less than the plan's in-network
