Therefore, before you receive care, it is recommended that you verify with the dental care provider that he or she is still contracted with the network.

**Limited to 3 individual $50 deductibles per family for Basic and Major Services. Then, after a waiting period Golden Rule pays 80% for Basic Services and 50% for Major Services. Basic Services and Major Services are defined in the policy. Depending upon the plan you select, reimbursement will be based upon either the network negotiated rate or the reasonable and customary charge.
