The Evidence of Coverage (previously titled Certificate of Coverage) for 2019 is written to make benefits coverage information easier to understand.
Information included in this PDF contains high-level overviews of the changes you and your clients can expect effective Jan. 1, 2020. Also included in the Evidence of Coverage (EOC) comparison is information on our new common Prescription Drug List (PDL), which aligns pharmacy coverage across PPO and HMO plans, allowing members access to the same pharmacy networks, PDL tiering and clinical rules. In addition, out-of-network benefit changes will take place for the following services on all UnitedHealthcare plans offered in California.
