Take steps to healthier living and join the Glucose Management Program

UnitedHealthcare is excited to introduce a new Glucose Management Program that creates a way for type 2 diabetics to establish healthy habits, and try new technologies and techniques that can help halt the devastating effects of the disease. Currently, over 38,000 members throughout the following markets have been invited to participate in the program – newly named Level2 – and take steps to healthier living:

• Texas

The Glucose Management Program provides participants with the latest in diabetes and fitness tracking tools to help encourage the pursuit of daily walking goals. Participants are invited to try state-of-the-art technology in diabetes monitoring that may provide opportunities for changes in dietary behaviors. These changes may help reduce the need for medication. Included as a part of the participant’s benefit plan, the Glucose Management program is provided at no additional cost to the employer or participant.

• Active program participants can use state-of-the-art technology in diabetes monitoring, such as the use of a Dexcom G6 continuous glucose monitoring system, which lets participants monitor their glucose levels 24 hours a day, 7 days a week.
• When our members join the Glucose Management Program by UnitedHealthcare, they are eligible to earn up to $730* a year in gift cards, a Fitbit Versa Lite Edition™ smartwatch for program use, a personal health coach and healthy living tips, all at no additional cost.

This program is available to all UnitedHealthcare plan members diagnosed with type 2 diabetes**.
