How to Enroll in Indiana PathWays for Aging

Get the health care you deserve.

Indiana PathWays for Aging is a Medicaid program that serves members who are age 60 and older. It is a long term services and supports program. Many people in this program also qualify for a Medicare Dual Eligible Special Needs Plan (D-SNP). 

This plan is available in all Indiana counties.

How to Enroll

Call the Indiana PathWays for Aging helpline at 1-877-284-9294 (1-87-PATHWAY-4). TTY users may dial 711.

In Person:

Visit a local FSSA office. Find an office near you.

If You Become Eligible for a Medicare Program

If a person becomes disabled, they may be eligible for Medicaid Disability. Some Hoosiers may be eligible for both Medicaid and Medicare. Hoosiers age 60 and up may have Indiana PathWays for Aging plus Medicare coverage. This Medicare coverage is called a Medicare Dual-Eligible Special Needs Plan (D-SNP). 

When members move from any other health coverage program to a Medicaid or a Medicare program, that start date may be in the past. When coverage changes, the benefits may also change. UnitedHealthcare is here to help you understand everything about your health coverage. We can answer questions, even if only one of your programs is with UnitedHealthcare. Calling Member Services is the first step to getting support and understanding options. 

Plan selection

  • Call the Indiana PathWays for Aging program helpline at 1-877-284-9294 (1-87-PATHWAY-4).
  • If you do not select a health plan, the State will assign one for you.
  • If you have both Medicare and Medicaid coverage, the State will attempt to assign the same health plan for both programs.
  • Other factors may be considered, such as your residential provider (if applicable).
