Disability Management Case Study: Marie
Bridge2Health offered crucial assistance for 60-year-old Marie, a drug store employee, diagnosed with chronic obstructive pulmonary disease (COPD). Along with having little understanding of her disease, Marie also had a history of mental illness, high blood pressure, obesity and smoking. She also was unable to work due to recurring bone fractures from osteoporosis and episodes of coughing.

Our Disability Specialist and Medical Case Manager worked together to help Marie formulate and implement a customized action plan. Marie was encouraged to proactively take control of her health care. She was given a blood pressure monitor to keep track of and control her symptoms, and she maintained a diary so she could provide her doctor with greater details surrounding her conditions.
Marie experienced great success. She not only quit smoking, but also lost a significant amount of weight. While her COPD still affected her health, she learned to manage her symptoms herself and as a result, required less frequent medical intervention. She also brought her blood pressure to a manageable level and returned to work. Marie says that with the assistance of her UnitedHealthcare case manager, she feels healthy and is happier than she has ever been. She believes that the intervention not only saved her life, but equipped her with the confidence and skills to become a more independent person
