    (UnitedHealthcare Insurance Company)
    Small business core plans based in Alaska provide access to any doctor at any facility within the Core network. Network and non-network cost share are the same for most outpatient services. Also, these plans have a combined deductible for both network and non-network charges. The Alaska Core network features Providence facilities in the Anchorage area and include providers in the lower 48 states. Also offered as an HSA option.
  • UnitedHealthcare Select Plus
    (UnitedHealthcare Insurance Company)
    Small business employers select plus plans based in Alaska provide access to any doctor at any facility within the Select Plus network without a referral. Network and non-network cost share are the same for most outpatient services. Also, these plans have a combined deductible for both network and non-network charges. The Alaska Select Plus network features Alaska Regional facilities in the Anchorage area and include providers in the lower 48 states. Also offered as an HSA option.
  • UnitedHealthcare Choice Plus
    (UnitedHealthcare Insurance Company)
    Small business employer, this is a traditional health plan with copayments, coinsurance and deductibles. Members have the freedom to see any doctor and visit any facility in our nationwide network, including specialists, without a referral and without choosing a primary care doctor. Members can choose services outside our network, but their payments will be higher. The Alaska Choice Plus network features Alaska Regional and Providence facilities in the Anchorage area and include providers in the lower 48 states.
  • UnitedHealthcare Options Non-Differential PPO
    (UnitedHealthcare Insurance Company)
    UnitedHealthcare Non-Differential PPO provides coverage for members who do not have standard access to the UnitedHealthcare PPO network service area. All care is reimbursed at a single coinsurance level. Members are not required to use a network provider. However, cost savings are available if members do receive care from a UnitedHealthcare PPO network provider.
