As required by the state of Arizona, the Health Care Insurer Appeals Process Information Packet must be provided to new enrollees upon enrollment. The packet includes information for plan subscribers if appealing a decision by UnitedHealthcare to not authorize or approve a service or pay for a claim.
Note: This notice applies to Arizona medical plan subscribers and to dental-only and vision-only plan subscribers.
The week of March 18, the packet was mailed to subscribers who did not receive it upon plan enrollment. Customers will be notified by mail of the upcoming subscriber mailing and packet.
