New enrollees in Arizona medical, dental-only and vision-only plans will receive a Health Care Insurer Appeals Process Information Packet. The distribution of this notice is required by the state of Arizona.
The packet will be mailed mid-May to subscribers that did not receive the packet upon plan enrollment. A mailing to customers prior to the subscriber mailing will include notice of the subscriber mailing and the packet. The packet includes important information about the process that plan subscribers should follow if they wish to appeal a decision by UnitedHealthcare not to authorize or approve a service or pay for a claim.
