Sweat Equity, the physical fitness reimbursement program, is being extended to members of New Jersey UnitedHealthcare large group plans (51+) starting Aug. 1.
There are more than 20 workout classes that qualify for reimbursement, from boxing and CrossFit to Zumba.
Eligible members1 can earn up to $200 for every 6-month period in which they complete 50 cardio-based workouts, including solo gym sessions, group exercise classes, a Citi Bike® ride and ClassPass activities.2
Members3 can start using the program when their medical policy renews or becomes effective in 2018. They must keep track of their exercises using the form provided and mail it to UnitedHealthcare with the required documents at the end of the 6-month period.

1Reimbursement benefit is available to the subscriber and the subscriber's covered spouse or domestic partner; no other dependents are eligible. For the subscriber's spouse or domestic partner to be eligible for this benefit, he or she must also be enrolled in a UnitedHealthcare product.
2Reimbursement is generally limited to the lesser of $200 subscriber/$100 covered spouse/domestic partner or the actual cost of the qualifying fitness costs per 6-month period, but the amount may vary by plan. Members should refer to their plan documents to determine eligibility and the benefit amount. The total annual reward amount for a member’s participation in incentive-based programs cannot generally exceed 30 percent of the cost of coverage. Rewards may be taxable. Members should consult with an appropriate tax professional to determine if you have any tax obligations from receiving reimbursement under this program. Eligible subscribers and their active covered spouses/domestic partners (no other dependents) may participate in this voluntary program.
3The Sweat Equity Program is a component of the fully insured medical benefit; there is no additional charge to members and participation is voluntary. The program is optional for self-funded groups.
