UnitedHealthcare and Arizona Care Network (ACN) are working together to provide a patient-centered health plan that helps promote better health and lower costs.
The NexusACO® plan will be available to Key Account employers with 51+ employees in Arizona with an effective date of July 1, 2019.
The new plan offers access to UnitedHealthcare’s Tier 1 network of physicians who coordinate all aspects of a patient’s care. The key to this framework is connecting the outcome-based results of high-performing providers, the better member experience of a primary care physician (PCP) to champion their care, and the incentive of a national network tiered on results and cost efficiencies to help deliver employer savings.
NexusACO delivers value to consumers and employers through 3 key features:
Better care coordination
NexusACO plan participants will collaborate with Tier 1 physicians through Arizona Care Network PCPs, including Dignity Health, Abrazo Community Health Network, Phoenix Children’s Care Network, and other quality primary and specialty care physicians.
Promotes better health outcomes
NexusACO provides access to care providers who proactively engage their patients more frequently, helping ensure at-risk patients receive timely, appropriate health screenings, and helping people manage chronic diseases such as diabetes, heart disease and arthritis.
Lower costs for quality health care
Employers who choose NexusACO may achieve savings on overall health care costs of up to 15%.1 These savings come from the plan’s emphasis on encouraging employees to use Tier 1 network care providers to coordinate their care.
