Important disclosure notice required for Maryland employers

An existing Code of Maryland Regulations requires insurers to provide Maryland customers a disclosure to include physician compensation, methods of paying physicians and premium dollar distribution at the time of plan selection. In addition, the disclosure must also be provided to Maryland plan enrollees at the time of their enrollment.
To meet the requirements, the disclosure will be included in your renewal kits and included with your plan documents on Employer eServices®. It can also be found on uhc.com.
The disclosure will be distributed and made available to your employees and enrollees through the following:
- Websites: uhc.com, myuhc.com, myuhcdental.com, myuhcvision.com.
- Included in the Annual Member Notice (AMN), which is a state specific notice to include instructions on where to find this notice.
Please download the disclosure and distribute it to your employees and enrollees at the time of enrollment.
