Arizona health care insurer appeals process information packet coming soon

As required by the state of Arizona, the health care insurer appeal process information packet needs to be provided to new enrollees upon enrollment. The packet includes important information about the process to follow should enrollees wish to appeal a UnitedHealthcare decision to not authorize or approve a service or pay for a claim. Packets were mailed out during the week of December 9 to subscribers who did not receive it upon plan enrollment. 

Note: This notice applies to Arizona medical, dental-only and vision-only plan subscribers.
