As required by the state of Arizona, health care insurer appeals process information, including information for subscribers who plan on appealing a UnitedHealthcare decision and notice to not authorize/approve a service or pay for a claim, must be provided to new enrollees upon enrollment.
This notice applies to Arizona medical plan, dental-only and vision-only plan subscribers.
Packets will be mailed out during the week of March 16 to subscribers who did not receive it upon plan enrollment. This is informational only – no action steps are required. Please retain a copy of the notice for reference.
