Safer Health Care: A Checklist for Patients

Do all you can to stay safe and be a smart patient. Print and use this checklist to get started.

❏  Keep a list of all the medicines, herbs and vitamins you take. Share it with each of your health care providers.

❏  When your doctor recommends a test or procedure, ask why you need it — and what the benefits and risks may be.

❏  When you pick up a prescription, check what’s in the bag. Make sure your name is on the bottle and that it’s the right medicine. If you’re not sure, ask the pharmacist.

❏  Ask for the results of tests and procedures. Don’t assume that “no news is good news.”

❏  Read all medical forms — or have someone read them to you. Be sure you understand them before you sign.

❏  Learn about any health conditions you have. Ask your doctor or nurse how to find reliable information.

❏  Speak up if there is any part of your health care you don’t understand. Keep asking questions until it’s clear.
