Utah rule for Access to Health Care Providers in Rural Counties
Effective July 22, 2020, the Utah rule R590-237 for Access to Health Care Providers in Rural Counties has been updated to identify changes to the lists of rural counties, independent hospitals, and federally qualified health centers. This rule allows members who reside in certain rural areas access to providers and facilities that may not participate in your plan’s network. This letter is informational only, no action is needed.
You may be entitled to coverage for health care services from the following providers if you live or reside within 30 paved road miles of the listed providers below, or if you live or reside in closer proximity to the listed providers than to your network providers.
The list of providers may change periodically, please check myuhc.com® or call the number on your ID card for verification. Please be advised that if you choose an out-of-network provider you will be responsible for any charges not covered by your health insurance plan.
If you have questions concerning your rights to see a provider on the list, you please call the toll-free member phone number on your health plan ID card. If we cannot resolve your problem, you may contact the Utah Office of Consumer Health Assistance in the Insurance Department, toll free.

- Counties with a population density of less than 100 people per square mile are each county in Utah except Cache, Davis, Salt Lake, Utah, and Weber.
- Rural counties where an independent hospital was built prior to December 31, 2000 include each county in Utah except Davis, Salt Lake, Utah, and Weber.
- Rural counties where an independent hospital was built after December 31, 2000 include each county in Utah except Cache, Davis, Salt Lake, Utah, Washington, and Weber.
- Non-contracting provider referrals to non-contracting providers are allowed in each county in Utah except: Cache, Davis, Salt Lake, Utah, Washington, and Weber.
