Is a Dual Special Needs plan right for you?

It might be right if...

 

  • You have Medicare and Medicaid
  • You have complex medical needs
  • You need help to coordinate your care
  • You would like extra benefits and features at no extra cost

It might be right if...

 

  • You qualify for either Medicare or Medicaid but not both
  • You do not receive Original Medicare benefits
  • You live outside an eligible plan coverage area

Dual eligible enrollees are either Full Dual Eligible or Partial Dual Eligible. Full Dual Eligible enrollees qualify for full stale Medicaid benefits, including covered medical care, whereas Partial Dual Eligible enrollees, don't qualify for medical benefits through their state Medicaid program, but may qualify for additional help. Check eligibility requirements before you start the enrollment process. Some D-SNPs only accept members who are Full Dual Eligible.

Types of Special Needs plans:

 

- Dual Special Needs plans (D-SNPs) for people who have both Medicare and Medicaid (called “dual eligible”)

- Chronic Special Needs plans (C-SNPs) for people living with severe or disabling chronic conditions

- Institutional Special Needs plans (I-SNPs) for people who live in a skilled nursing facility

- Institutional-Equivalent Special Needs plans (IE-SNPs) for people who live in a contracted assisted living facility and need the same kind of care as those who live in a skilled nursing facility

You are not required to use Optum Home Delivery for a 90-/100-day supply of your maintenance medication. If you have not used Optum Home Delivery, you must approve the first prescription order sent directly from your doctor to Optum Rx before it can be filled. 3 Vision benefits vary by plan and are not available with all plans. Additional charges may apply for out-of-network items and services. Yearly routine eye exam and $100–400 allowance for contacts or designer frames, with standard (single, bi-focal, tri-focal or standard Tier 1 progressive) lenses covered in full either yearly or every two years. Limitations and exclusions apply.
