Natharan is grateful for HouseCalls visits

Natharan is enrolled in a UnitedHealthcare Dual Special Needs plan, a Medicare Advantage plan that works with Medicaid to help her manage her overall health. Among other benefits, she has come to appreciate the yearly visit provided by a HouseCalls practitioner.

“It was very simple,” Natharan said. “(My HouseCalls nurse practitioner) will stay about an hour maybe, you know, and just sit and talk to me about whatever's going on with my body.”

Advanced care practitioners — who are licensed and trained nurse practitioners or physician assistants — bring health check-ins directly to members’ homes, like Natharan, with HouseCalls, at no extra cost.

These visits usually include:

  • A yearly in-home visit with a licensed clinician
  • Physical exams, screenings and reviews of medication
  • Help connecting to social services in the community, if needed (for example, food, shelter and transportation)
  • A summary sent to the member’s  primary care provider

HouseCalls helps make connections to primary care, while making preventive care more accessible, all in the comforting environment of home. For Natharan, a HouseCalls visit has meant a great deal.

“It’s kind of unusual to get that much time with a health care provider,” she said. “It’s personal.”
