Image Content Requiring Text Alternatives:

  • Controls, state indicators, CAPTCHA
  • Charts, graphs, diagrams, illustrations

Presentation that should not have text alternatives:

  • Lines, borders, boxes, frames, bullets, and spacer images
    • Example: "blue arrow" describing styling for links in bulleted list is unwanted alt text for what is considered decorative content

    • Exception: Images used specifically to set the mood

Resize Text

Can users with poor vision double text size on the page without losing functionality or content?

  • Does text on the image get blurry?

Simple screen zoom (magnification) often provides necessary support for in-page text.

Summary of Text Equivalent Approaches

Preferred: Visible text near (or associated with) image

  • Try to always have text within the page rather than in images because in-page text is immediately available for all users
  • Add labels next to icons (headers, buttons, controls- such as search or help)
  • Add a link to another web page to provide longer descriptions for complex images where alt text field is not sufficient.  

Controls, Input, and Icons

The App Store and Google Play images above are considered link controls, since clicking on them take you to the respective app stores. When an image is coded as a link, the alt text is announced instead of aria label (alt text overrides aria label).

Try it out with your screen reader. Add both alt text and an aria label to hear how it is announced. Make sure that our screen reader users know where they are going since there is no link text for these images.

Innovation in Advancing Health Equity Award

 The National Business Group on Health, a non-profit association of 420 large U.S. employers, honored UnitedHealth Group (UHG) at the National Business Group on Health’s April 2018 Business Health Agenda conference with their “Innovation in Advancing Health Equity Award”. UHG received the award based on our work to capture social determinants of health (SDOH) and other non-traditional data to integrate care and connect members with services. Current initiatives focus on the Medicare population and integrating SDOH data for a more complete view of a member’s needs, to match members with government and community resources that improve care outcomes and provide whole person support. UHG is also working to make recommendations to the Centers for Medicare/Medicaid to expand the diagnostic code set (ICD-10) to include social determinants such as lack of transportation, food insecurity, and utility and telephone assistance.

Complex Images

The image above contains complex content where a long description is needed. For example, alt text="WESTMED ACO Change in Utilization and Costs, 2011 to 2012 chart" would not be sufficient, as vision impaired users would not have the same experience. Instead, we chose to place the text as bullet points under headings directly in the text and mark the image as decorative.

This complex image is from the 2nd FAQ drawer on the Value Based Care Results page.

Decorative

The image above has text that is identical to the heading in the content. Providing alt text would result in a redundant announcement, so the image is considered decorative in this case. A user can still understand the information without the image present. One can also think of the heading as a caption for the image, as the text is nearly identical.

Text equivalents that are simple descriptions of images are probably decorative elements, and should not have a text equivalent. Examples are: a woman shaking a man's hand, a clock next to a heart, blue arrow, or blue border.
