NARRATOR: To begin, open a web browser, and type in www.unitedhealthcaremotion.com

The UnitedHealthcare Motion website appears. Part of the content, near the start, is a “sign up” button.

NARRATOR: Once the site loads, click the sign-up button. You can sign up using your insurance plan group number, which can be found on the front of your health plan ID card, or use your social security number.

The sign up form is shown on screen with the heading “Sign Up for Motion.”

NARRATOR: For some, you may also receive a communication with a registration number to use as a third sign-up option. In this demonstration, we'll register using the insurance plan group number.

A drop down list with three options appears. "Insurance Plan Group Number" is selected.

NARRATOR: Enter the number into the group and policy number field.

The number is filled into the blank.

NARRATOR: Enter your first and last name...
On the screen the first and last names are entered into the form.

NARRATOR: Select your date of birth...
January 1, 1957 is selected from the drop down menus.

NARRATOR: And click the sign-up button.

The "Sign Up" button is selected.

The "Create your login" web page is shown.

NARRATOR: If you have entered the information correctly, it will bring you to the create your log-in page. Please make sure that your email address is correct. If it's not, you can hit the change button to enter a different email address.

NARRATOR: Then enter a new password for this account.

The "password" blank is filled in with black circles for each character.

NARRATOR: You'll use your email and password to log in to the site going forward. Be sure to review the terms of use and privacy policy and then check the box.

NARRATOR: Finally, click "create account." A congratulations message confirms you successfully created your account.

A congratulation message pops up. The message mentions that a $55 registration credit is available for purchasing an activity tracker.

NARRATOR: You may have noticed there's a $55 registration credit to select a device. If you need to purchase a device, select "shop activity tracker."

NARRATOR: Then purchase a tracker that works for you. Be sure to click on "more" for more details on the device.

NARRATOR: Those who select Apple Watch have an additional payment option, which is known as "walk it off."

The "Walk it off" button is highlighted with a red box around it.

NARRATOR: This option allows members to make an initial payment and walk off the remaining balance of the device over a 6 1/2-month time period. If you ordered a device, plan to pair it once you receive the device in the mail.

The Walk-It-Off payment option currently applies to the Apple Watch only. You cannot have any existing outstanding tracking device balances with Motion. You can only purchase one Apple Watch, under the Walk-It-Off payment option, at a time. Your total price = device price + administrative fee + taxes and shipping and handling. As you achieve your daily Frequency, Intensity, Tenacity (FIT) goals, your accrued monthly rewards will be applied toward the outstanding balance for your Apple Watch. The initial payment due at checkout will include administrative fee, taxes and shipping and handling. Any outstanding balance after 6.5 months will be billed to your stored credit card on file. Your credit card information is stored to facilitate automated billing. You can make changes to or update your credit card information at any time by visiting your profile page at UnitedHealthcareMotion.com. At that time, the Walk-It-Off option will be complete. At the 90-day mark, you must meet a weekly average of 3 FIT goal completions (any FIT goal combination); otherwise the outstanding device balance will be applied to the stored credit card on file. At that time, the Walk-It-Off option will be complete. Device returns can only be made within 14 days from the purchase date. Specific return criteria applies. If your Apple Watch is deemed to be defective, please contact Motion Member Services for additional details 1-855-256-8669. Device balances left unpaid will be secured from future FIT earnings until balances are paid in full.
