The 2027 Level Funded portfolio introduces expanded benefits, enhanced specialty care programs and market growth opportunities, plan customization and benefit buy up options for 51+ previously announced here, with quoting available Sept. 1, 2026, for Jan. 1 2027, effective dates.

Key updates include:

Expanded benefits across UnitedHealthcare Level Funded, Oxford Level Funded and Surest Level Funded plans, including:
Dental anesthesia for plan participants under the age of 9
Doula services
Statewide embedded infertility in RI and NY
Specialty care support through the Level2 program for plan participants with Type 2 diabetes

Expanded benefits across UnitedHealthcare Level Funded and Oxford Level Funded plans include

TMJ services
HSA plan updates with zero-dollar virtual visits
Wig coverage
IRS-driven minimum deductible updates
Physical therapy, occupational therapy, and speech therapy visit limits lifted for mental health and substance-related and addictive disorders

Surest Level Funded plans expanded benefits include:

Cervical biopsies, colposcopy procedures at $0 member cost share
UHC Store is now available for all members

Network expansion

UnitedHealthcare Level Funded only:
- California removing Core Essential for 101+
- Louisiana adding NexusACO for 2-300+
- Nebraska removing Choice for all group sizes
- Washington adding Doctors Plan for 2-300+ in 6 counties (Clark, King, Mason, Pierce, Snohomish, Thurston)

Surest Level Funded county expansion in Wisconsin opening up the entire state.

Specialty savings opportunities:

New specialty savings opportunities for UnitedHealthcare Level Funded, Oxford Level Funded and Surest Level Funded, including a limited-time¹ credit of up to $5,000 for eligible 2–50 groups that add dental and select specialty products.
