Daphné provides patient-centered in-home care, one member at a time

DAPHNE: I am a certified registered nurse practitioner.

In the home setting, you're able to really see the barriers patients have.

I am Daphne. I am an advanced practice clinician.

I have the privilege of seeing the member in their home, meet them where they are at. I get to see a lot more than you would see in an office setting.

Any chest pain, any shortness of breath?

FEMALE: And she came in with her nursing experience, but she came in so kind, unassuming,

and I think that's what helped our relationship grow because it was almost like a friend. But she is so knowledgeable of her profession, that I get to get some of that good stuff that she knows on how to better take care of myself.

DAPHNE: Right now, I'm helping your flexion and your mobility.

We collaborate with the doctors. We engage with other team members to make sure the patient has everything they need.

And when I see them on their monthly or quarterly visits, I'm able to make sure there aren't gaps that need to be filled

as far as preventative care, medications that need to be refilled.

FEMALE: She monitors that on a monthly basis. Off the record, she'll check on me on, hey, I'm just checking in with you. And that's rare.

DAPHNE: I love serving the community. I love being the liaison, the bridge between them and their doctors. When you work with somebody and you see how far they've come, and I'm proud to be a part of that.
