Rocky Mountain Health Plans - RAE


Health First Colorado, Colorado's Medicaid program, is the Medicaid plan for children (newborn to age 18), adults age 19 to 65 and pregnant women who live in Colorado and meet income and other requirements.

Members are assigned to a Regional Accountable Entity (RAE) (sometimes called Regional Organization). Rocky Mountain Health Plans (RMHP) serves members in RAE Region 1 (Anyone with a primary care provider in the following counties: Archuleta, Delta, Dolores, Eagle, Garfield, Grand, Gunnison, Hinsdale, Jackson, La Plata, Larimer, Mesa, Moffat, Montezuma, Montrose, Ouray, Pitkin, Rio Blanco, Routt, San Juan, San Miguel, and Summit).

Rocky Mountain Health Plans - PRIME


Rocky Mountain Health Plans - PRIME is a Medicaid Managed Care plan for adults or those with a disability who live in certain counties on the Western Slope of Colorado and meet income and other requirements.

Rocky Mountain Health Plans (RMHP) - Child Health Plan Plus (CHP+)


Through the Child Health Plan Plus (CHP+) program, Rocky Mountain Health Plans (RMHP) provides low-cost health insurance benefits to children and pregnant women 19 and older who do not qualify for Health First Colorado but do not earn enough to pay for private health insurance.
