Rocky Mountain Health Plans - RAE


Health First Colorado (Colorado's Medicaid program) is the Medicaid plan for children (newborn to age 18), adults aged 19 to 65 and pregnant Members who live in Colorado and meet income and other requirements. Every Health First Colorado (Colorado's Medicaid Program) Member belongs to a regional accountable entity (RAE), (sometimes called regional organization). 

Rocky Mountain Health Plans (RMHP) ), A UnitedHealthcare Company, is the RAE (Region 1) for Health First Colorado (Colorado's Medicaid Program) Members who have a primary care provider that is located in the following counties: Archuleta, Delta, Dolores, Eagle, Garfield, Grand, Gunnison, Hinsdale, Jackson, La Plata, Larimer, Mesa, Moffat, Montezuma, Montrose, Ouray, Pitkin, Rio Blanco, Routt, San Juan, San Miguel, and Summit. Rocky Mountain Health Plans is the UnitedHealthcare Community Plan for Colorado.

RMHP helps to coordinate your physical and behavioral health care, to include assessments and referrals, access your benefits, make sure you get the help you need - when you need it, work toward health and wellness goals to live healthier lives. Rocky Mountain Health Plans - PRIME


Health First Colorado (Colorado's Medicaid program) is the Medicaid program for Members who live in Colorado and meet income and other requirements. Rocky Mountain Health Plans (RMHP) - PRIME is the Medicaid Physical Health Managed Care plan for adults or those with a disability, who live in one of the following counties: Delta, Garfield, Gunnison, Mesa, Montrose, Ouray, San Miguel, Rio Blanco and Pitkin, and meet income and other requirements. Rocky Mountain Health Plans is the UnitedHealthcare Community Plan for Colorado.

For behavioral health benefits, RMHP is also the regional accountable entity (RAE), (sometimes called regional organization) for Health First Colorado (Colorado's Medicaid Program) Members who are on PRIME.

Through the Child Health Plan Plus (CHP+) program, Rocky Mountain Health Plans (RMHP) provides low-cost health insurance benefits to children and pregnant Members 19 and older who do not qualify for Health First Colorado but do not earn enough to pay for private health insurance. 

RMHP is the CHP+ physical and behavioral health Managed Care plan for Members who live in one of the following counties: Archuleta, Delta, Dolores, Eagle, Garfield, Grand, Gunnison, Hinsdale, Jackson, Lake, La Plata, Mesa, Moffat, Montezuma, Montrose, Ouray, Pitkin, Rio Blanco, Routt, San Juan, San Miguel, and Summit.
