Choice Plus offers broad, national access to physicians and hospitals, out-of-network coverage and no referrals needed to see a specialist.
Core Essential plans offer tailored local access to physicians and hospitals, network-only coverage and do not require referrals to see a specialist.

OCI HMO plans offer regional access to physicians and hospitals, network-only coverage and a primary doctor to coordinate care and refer specialist services.
