With UnitedHealthcare Navigate Plus® health plans, you get the highest level of coverage when you get a referral from your primary care provider (PCP) before seeing another network physician. You have a lower level of coverage if there is no referral. You have the lowest amount of coverage for out-of-network providers.
With Navigate Plus, PCPs get to know you and your medical history. They help coordinate care across a wide spectrum of health services and make sure you're getting the care you need when you need it.

Navigate Plus plans provide well-rounded coverage for many services including:
