- Freedom Plan® PPO
Our Freedom Plan PPO products offer combined in- and out-of-network coverage, do not require referrals for specialist visits and include an in-network deductible and coinsurance. This plan is offered with the Freedom Network but also provides access to the national UnitedHealthcare Choice Plus Network when outside the Oxford service area.
- Liberty PlanSM PPO
Our Liberty Plan PPO products offer combined in- and out-of-network coverage, do not require referrals for specialist visits and include an in-network deductible and coinsurance. This plan is offered with the Liberty Network but also provides access to the national UnitedHealthcare Choice Plus Network when outside the Oxford service area. - Oxford® EPO (Freedom Network/Liberty Network)
The Oxford EPO is an in-network only product and can be purchased with either the Freedom Network or the Liberty Network. These plans do not require a referral for specialist visits and provide access to the national UnitedHealthcare Choice Plus Network when outside the Oxford service area.

Our Liberty Plan Gated PPO products offer combined in- and out-of-network coverage and a referral for specialist visits. This plan is offered with the Liberty Network but also provides access to the national UnitedHealthcare Choice Plus Network when outside the Oxford service area.
- Liberty PlanSM Gated EPO
Our Liberty Plan Gated EPO is an in-network only product and requires a referral for specialist visits. This plan is offered with the Liberty Network but also provides access to the national UnitedHealthcare Choice Plus Network when outside the Oxford service area.
- Oxford® PPO HSA (Freedom Network/Liberty Network)
The Oxford PPO HSA is a consumer-driven health plan that is made up of two parts: a high deductible health plan and a Health Savings Account (HSA). With this plan, members can open and deposit money, before taxes, into an HSA, which is their own personal savings account. Members can use the money to pay for eligible medical and pharmacy expenses, including their deductible, or save it for future expenses. This plan also gives members the choice to go in- or out-of-network for care, and referrals are not required for specialist visits.
- Primary AdvantageSM (Liberty Network)
Primary Advantage is an in-network only product that puts an emphasis on benefits where health care encounters are most prevalent: preventive care, services provided by a primary care physician (PCP) or OB/GYN, and Tier 1 prescription drugs, which are available through low copayments and not subject to a deductible. If a higher level of care is required, Primary Advantage provides members access to comprehensive coverage through a vast network of specialists and hospitals. These services are subject to a deductible; however, once the deductible has been met, benefits are covered through clearly defined copayments, a feature designed to add predictability to health care spending.
- Oxford® EPO HSA (Freedom Network/Liberty Network)
The Oxford EPO HSA is an in-network only consumer-driven health plan that is made up of two parts: a high deductible health plan and a Health Savings Account (HSA). - LibertySM HMO
The Liberty HMO is a great option for those who like to stay in-network and have their care coordinated by a primary care physician (PCP). Liberty HMO plans allow members to receive covered services from the in-network primary care physician of their choice. Members are required to obtain a referral for specialist visits.

- UnitedHealthcare Core
UnitedHealthcare Core plans give members the freedom to choose from any health care professional in the UnitedHealthcare Core Network, including specialists, without a referral or choosing a primary care physician (PCP). Since Core is an open access product, members can seek care from any provider, but they may pay more out-of-pocket costs when they do not seek care from network providers or facilities. - UnitedHealthcare Core Essential
UnitedHealthcare Core Essential plans give members the freedom to choose from any health care professional in the UnitedHealthcare Core Network, including specialists, without a referral or choosing a primary care physician (PCP). Since this is a network-based product, members must seek care from network providers for benefits to be covered. - UnitedHealthcare Charter
UnitedHealthcare Charter offers network coverage for members who obtain a referral from their primary care physician prior to seeing another physician or specialist in the UnitedHealthcare Charter network. No coverage is provided without a referral. Non-network coverage is not available, except in the case of emergency care. - UnitedHealthcare Charter Plus
UnitedHealthcare Charter Plus provides the highest level of coverage when the member has a referral from their primary care physician prior to receiving services from another network physician, and a lower level of coverage if there is no referral. The lowest level of coverage applies if a non-network provider is used. Members enrolled in these products have access to the UnitedHealthcare Charter Network. - UnitedHealthcare Choice Advanced
UnitedHealthcare Choice Advanced gives members the freedom to choose from any health care professional in the UnitedHealthcare network, including specialists, without a referral or choosing a primary care physician (PCP). Members can save money when they make more informed decisions about the providers they choose, selecting physicians that have been recognized for delivering quality, cost-efficient care as well as certain lower-cost facilities. Since Choice Advanced is a network-based product, members must seek care from UnitedHealthcare Choice network providers to receive benefits. - UnitedHealthcare Choice Plus Advanced
These are open access plans that encourage members to seek care from certain providers to get the most value for their health care dollars. Choice Plus Advanced plans offer access to our national UnitedHealthcare provider network while also providing out-of-network coverage. By making informed decisions about the facilities they choose, members can save money by going to certain network providers and facilities that are less expensive and still deliver the same quality care as other network providers. Members can choose services outside our network, but their payments will be higher.
